SurveyGizmo is running a contest to give away a free ipod when someone signs up for a free account this month - no credit card needed or anything like that.
This is by far the best online survey application I have ever seen. It blows the doors off SurveyMonkey and it has a free level that might be great for most people. You can use it to create surveys, polls, contact forms, data collection, landing pages, you name it. If it’s got a form field in it they can do it.
I know the guy who built the app and he’s on the up-n-up. He is really open to suggestions — you can always let him know what you think. He responds to bugs or feature requests (when they’re a good idea!) lightning fast.
